Policy Acknowledgement Form

I, the Employee, have received a copy of the Company’s Employee Handbook containing Company policies (the “Handbook”) dated [Insert Date] and I have read it, understood it, and agree to comply with it in letter and spirit.
I understand that Company has the maximum discretion permitted by law to interpret, administer, change, modify, or delete the rules, regulations, procedures, and benefits contained in the Handbook at any time. No statement or representation by a supervisor or manager or any other employee, whether oral or written, can supplement or modify this Handbook. I also understand that any delay or failure by Company to enforce any rule, regulation, or procedure contained in the Handbook will not constitute a waiver of Company’s right to do so in the future.
I understand that neither this Handbook nor any other communication by a management representative or any other employee, whether oral or written, is intended in any way to create a contract of employment. If I have a written employment agreement or other such agreement for engagement by the Company which is signed by an authorized Company representative, and in case this Handbook conflicts with the terms of such agreement, I understand that the terms of the agreement will supersede this Handbook.
COMPANY RESERVES THE RIGHT TO MAKE CHANGES TO THIS HANDBOOK FOR THE PURPOSE OF MODIFYING, REVISING AND UPDATING COMPANY POLICIES OR THIS HANDBOOK. NOTICE OF CHANGES WILL BE PROVIDED TO EMPLOYEES PERIODICALLY AND SUCH CHANGES FORM A PART OF THIS HANDBOOK. VIOLATION OF ANY COMPANY POLICY MAY RESULT IN TERMINATION OF EMPLOYMENT.
